[LON-CAPA-users] Issue with Configuration of 'Request Creation of Courses'

All,
I am trying to configure our system to show the Course Creation form in our development environment, and keep getting the following error. Leading up to the error, here is what I'm doing: Set domain configuration => Request creation of courses => select any User affiliation (for any category) => Save Changes.
Ideas?
