That does not happen here (under LON-CAPA version 2.10.1).

Hello all;

About 1 out 10 times a problem is rendered, content inside all <m></m>
tags shows up as "[TeX unconverted due to previous errors]".  After
clicking "New Randomization" everything displays fine.

In the web server logs, the only possibly relevant error is

        (Missing operator before E_TRIE_MAXBUF?)
Bareword found where operator expected at (eval 119198) line 1, near
"@E_TRIE_MAXBUF".

Tags that use <m display="mimetex"></m> always show up fine.

An example problem that I can confirm has this issue can be found here:
http://ftfl.ca/misc/bone_mineral_density.problem.

I'll guess the overhead to display an image for every piece of LaTeX
(display="mimetex") is much higher than rendering a font
(display="tth"), so always using mimetex might not be the best
workaround?
