at a minimum you need,

loncapaid, domain, pcr, status

status is 'correct_by_override' (it could be another value, but it is
unlikely and only useful in strange situations.  (Ie
'incorrect_by_override', 'excused' etc)

pcr is a floating point number between 0 and 1 that is the percentage
correctness for the problem (I.e if it is a 4 point problem and they
are getting 3 points, this should be a 0.75)


If you have multiple parts in the problem you can have multiple pcr
and status


If you really want you can also upload a tries value.
