[LON-CAPA-admin] Strange issue with submission

There is something strange going on, and I need help. This may be a significant bug hidden
somewhere or maybe a one-time fluke.
We have a problem that asks the student to calculate their grade as a percentage given some
fake data. The student initially did not enter the grade using a percent sign. Fine - she has two
trials that are wrong.
She then enters the correct answer 79.8%, but does not get recognition that her answer is right
from LON-CAPA. The number of tries does not increment either. Try 3 was “approx_ans” but her
problem stays with 2 tries.
The grading chart shows several trials and then a whole bunch of attempts
to hit the submit button. Listed after trial 2 is trial 3, but listed right after that is trial 2 again.

Any insights? This one is a bit disturbing. Is this a poorly coded problem causing issues or something
special with the % sign and how the browser is interacting?
