[LON-CAPA-admin] Map not loaded

Hi,

Yesterday (~3pm) users in the fsuK12 could not access their courses. The
message shown is "Map not loaded: The file /home/.... does not exist." It
doesn't matter which access server the user is directed to. This seems to
indicate the library server is not passing the sequence over. The logs on
the lib server does not show anything to suggest what the problem might be.
Later yesterday (~6pm), some users were able to access their courses. This
morning, one of the teachers cannot access one of his courses - when he
click on the select button, it brings him back to the roles page.

Any idea what is going on and how to fix the problem?
