[LON-CAPA-admin] Randomized exam grading is misbehaving - badly

There seem to be a number of problems with randomized exam grading. 
Here is the list.

1. There are 5 questions. It grades only 3 for some students. The 
grade checking flags 3 students with missing bubble for questions 4 
and 5 but the score chart shows almost a third of the students (~60) 
with q 4 and 5 missing.

2. The grading is different when I login as dc and then select the 
course (going via /adm/login) compared to when I login as cc 
(different username) either via the FSU cas or thru /adm/login. As 
dc, all 183 students are graded but see point 1 whereas as cc only 80 
students are graded. The file, scantron_corrected_<quiz name>.txt is 
a html file that is the cas login page.

3. The codes for grading are saved but if I use the codes none of 
them match with the students. Of course, I then select the blank - 
then it grades but with the results as in point 1 and 2.

I have tried versions 2.2.2, 2.3.0 and 2.3.1. Point 2 seems to 
suggest that somehow cas has mess things up. This is a show stopper.
