Hello!

We are in the process of updating our user database for LON-CAPA.  


In order to facilitate this process, I need the following information:

	name
	title
	institution
	discipline/subject 
	mailing address
	phone number
	current system (CAPA, Lecture Online, LON-CAPA - which system)
	future user of LON-CAPA (estimated implementation)
	no longer a user (brief explanation) (remove from mailing list?)
	Comments
