A comment in that bug report includes information about the version of default_homework.lcpm you can use with 2.11.2 if you encounter this (rare) bug, and want to prevent it.

I apologize, because I think this might have gone by before, but we have a student running a version of a problem
where we get the following error:
Usage: random_set_seed(@seed)
@seed[0,1] must be two integers in the range (1,1) to (2147483562,2147483398) and usually comes from a call to random_get_seed() or random_seed_from_phrase($phrase). at /usr/local/lib64/perl5/Safe/Hole.pm line 68 This occurred while running:
Is this one of those 1 in 2147483562 kind of errors? Is there a patch or workaround for this?
