[LON-CAPA-admin] will not initialize role

No idea of the causes... but I seem to recall that when this occured
there was generally a file of the type
/home/httpd/perl/tmp/username_domain_courseid_parms.db
which needed to be (re)moved before the user could initialize the
particular course again on the given server.
