[LON-CAPA-admin] scantron testing

Hi Nathan,

Well, it's called Scantron, but we are also using NCS.

The form is NCS MM226157-1 in pumpkin color, but they come in all  
kinds of colors.

I have attached a scan of the front and back of the form - note the  
"Code" next to the "Section" field.
